QI Training & Applied Learning Support
If you have a QI project idea in mind, completion of this application will help us to determine the level of support you are looking for based on your time commitment, level of interest in QI education, coaching requirements, etc. Information gathered from these applications regarding the focus of your quality improvement idea are forwarded to a PQI representative in your area for initial review to support identifying opportunities for alignment and resources available.

Your application will be shared within the IH Quality Portfolio as needed to administer the Education/Applied Learning Support program and will be shared externally with members of the IH PQI governance committee. Participation is voluntary and by submitting the form you consent to this use.

IH PHYSICIAN QUALITY IMPROVEMENT - LEARNING OPPORTUNITES
LEVEL 1: Introduction to Quality Improvement (Online Learning)
Institute for Healthcare Improvement (IHI) Open School - Physicians are provided a license to IHI Open School to complete introductory CME accredited QI course work, supported by 5 hours of sessional time.

LEVEL 2: Introduction to Quality Improvement (Facilitated Workshop)
Education sessions (virtual) facilitated by PQI staff and physicians, provide an interactive opportunity for participants to learn and work with their peers. The workshop highlights the first steps in developing a project idea into a project plan.
Sessional funding is provided to support participation.

APPLIED LEARNING SUPPORT:
Prerequisite - Introductory QI education complete (Level 1 or 2).
- Submit an application with a QI project idea that you would like to spend a 1 - 3 month period (approx. 15 - 20 hrs) developing.
- Sessional support for time spent developing project
- PQI Team resources (PQI Consultant, Data Analyst, Privacy & Security, etc.)
LEVEL 3: Advance Quality Improvement Education - Cohort training
- Applied learning – Develop your QI idea into a project plan during this 9-10 month program
- “Real time” coaching from PQI Consultant and team
- Stakeholder Engagement / Participation - Dyad Partnerships
- Sessional Support for Education and Project Development
- Access to dedicated resources, including data analysis, IMIT, Privacy Consultant & more!
- Bring a friend! Group projects supported.
ALUMNI SUPPORT: Post PQI Graduate
- Submit application outlining your new QI project idea
- Sessional support for time spent developing project
- PQI Team resources (PQI Consultant, Data Analyst, Privacy & Security, etc.)
