Lung Disease and Conditions Voice of the Patient Survey

Lung Disease and Conditions Voice of the Patient Survey

Introduction

Drug and health technology agencies  want patients and caregivers to contribute to the province’s drug review process. The committees that evaluate drugs would like to understand the real impact that disease and treatment has on the daily lives of people.

Patients and caregivers can contribute to this process by sharing their personal experiences. When a drug is up for review, registered Patient Advocacy Groups can make a submission on behalf of a group of patients. The goal is to help the Committee understand the personal implications of the illness and its treatments.

The Lung Health Foundation is pleased to provide a voice for patients and their caregivers.  The following is the Voice of the Patient survey.  This survey is used to collect information from individuals living with a chronic lung condition and their caregivers. We would like to know how your lung condition affects the quality of your life.

All information received will be summarized and submitted to the Canadian Agency for Drugs and Technologies in Health.  This may inform their consideration of drugs intended to treat lung conditions.

To include your voice, please complete this survey. Please feel free to share the link to this survey with anyone you know who is living with or caring for someone with a chronic lung disease / condition.

If you have any questions or further comments please call the Lung Health Foundation’s Lung Health Line at 1-888-344-LUNG (5864).
We would like to thank you for agreeing to participate in this important survey.  All personal information will be held in the strictest confidence and will not be shared with third parties without your prior consent to do so or otherwise required by law.

The information you voluntarily provide in this survey will be used to better inform our submissions to the Canadian Agency for Drugs and Technologies in Health. Your responses will be combined with those of many others and summarized in a report to further protect your privacy. No personally identifiable information will be shared within the submissions.
