The documents that should have been sent with this application are:

1. A digital PDF copy of your Curriculum Vitae (Question #1)
2. Digital PDF copies of Letters of Recommendation (Letter #1 and Letter #2)
3. A letter describing how you how you plan to use the BC TEAL/TESOL Award. (Question #4)
4. A letter describing: (Question #5)

A history of service to the local/provincial association or affiliate
The need for financial assistance for travel to the annual conference/convention
The extent to which attendance at an annual conference/convention will benefit the local/provincial ESL/EFL community
