Patient Experience Survey for Cowichan

The purpose of this survey is to better understand whether the healthcare needs and preferences of the people who live in Cowichan Valley are being supported. Your feedback will help us understand what’s missing and how we might make it better.
Completing this survey is voluntary. All your answers will remain anonymous. Survey responses will be summarized so that no individuals are identifiable in the final results.
Results will be used by the Cowichan Primary Care Network – of which all Cowichan family practice clinics are a part – to continue our work on improving access to health services.
Everyone who completes the survey and who provides their email address will be entered in a draw to win a gift basket (estimated value of $100). The first draw will take place on December 20, 2021. The next draw will take place on January 31, 2022.
