A maximum 100-word description of how you plan to use the Health Education Award (Question #3)
A description of the goals and rationale for the project (Question #4)
A description of the timeline for completion for the project (Question #5)
A budget specifying anticipated expenses for the project (Question #6)
A review of research or a record of practical experience to support the project or use of funds (Question #7)
