Pediatric Outpatient Survey

NL Health Services is committed to providing quality care to patients and their families. The Pediatric Department is conducting a Patient Satisfaction Survey. Information collected will be used to help determine ways in which the Pediatric Department can improve its services for the children and families in the Central Zone.

This short survey should take approximately 5-10 minutes to complete. Your participation in this survey is voluntary, and your decision to participate will have no impact on the care you receive from NL Health Services. Please be assured that your responses will be anonymous and held in strict confidence.
