Purpose of the Survey
Western Health is committed to providing quality care to patients and their families. Western Health is a Person- and Family-Centered Care organization, which means working together with patients, families, health care providers, staff and volunteers to provide respectful, compassionate, quality and safe care.

This survey asks about your recent experience with the Perioperative Program. The results will be shared anonymously with health care providers and leadership to improve the quality of services.
