About the Survey
At NL Health Services - Central Zone, we are committed to providing quality care to patients and their families. NL Health Services is a People Centred Care organization, which means working together with patients, families, health care providers, staff, and volunteers to provide respectful, compassionate, quality and safe care.
This voluntary questionnaire asks about your perceptions of care. This information is confidential and is designed so that it is not connected in any way to you personally or to the services you receive. Please do not use any names or identifying information in the responses. Please see the Privacy and Confidentiality notice below for more details.
Anything you would like to tell us will be used only for the purpose of making the quality of care better for our clients. We want to continue to improve our care here every day and your experience will help us do that. The information is being collected to help our programs identify areas of strengths on which to build, and areas for improvement. Your feedback is important and will ultimately help to enhance the overall mental health and substance use system in Newfoundland and Labrador.
