About the Survey
At NL Health Services - Central Zone, we are committed to providing quality care to patients and their families. NL Health Services is a People Centred Care organization, which means working together with patients, families, health care providers, staff, and volunteers to provide respectful, compassionate, quality and safe care. 

This voluntary questionnaire asks about your perceptions of care. This information is confidential and is designed so that it is not connected in any way to you personally or to the services you receive. Please do not use any names or identifying information in the responses. Please see the Privacy and Confidentiality notice below for more details.

Anything you would like to tell us will be used only for the purpose of making the quality of care better for our clients. We want to continue to improve our care here every day and your experience will help us do that. The information is being collected to help our programs identify areas of strengths on which to build, and areas for improvement. Your feedback is important and will ultimately help to enhance the overall mental health and substance use system in Newfoundland and Labrador. 

It takes most people between 10 and 20 minutes to complete this survey. Not all items will apply to everyone. If a question does not apply to you, please check the 'Not Applicable' box rather than leave the answer blank. You may exit the survey at any time. If you exit the survey before finishing, information will be kept for questions you have already answered. You must use the ‘back’ button and delete any responses to ensure they are not recorded.

Your participation is voluntary. NL Health Services prioritizes privacy and confidentiality for all participants, clients and partners. This survey is designed to be anonymous. Responses will be available to the Evaluation Division and managers of the Mental Health & Addiction Services' program (as listed above). Reports and/or recommendations developed from the combined results may be shared with staff and leadership within NL Health Services. All information collected is stored in a secure office and/or on a secure network belonging to NL Health Services. If you have concerns about your privacy in the completion of this survey you may contact: myvoicenl@nlhealthservices.ca.
