Purpose of the Survey
At NL Health Services - Central Zone, we are committed to providing quality care to patients and their families. NL Health Services is a People Centred Care organization, which means working together with patients, families, health care providers, staff and volunteers to provide respectful, compassionate, quality and safe care. This survey asks about your recent experience when receiving a day surgery procedure. This is an opportunity for you to provide feedback.

Privacy and Confidentiality
NL Health Services prioritizes privacy and confidentiality for all participants, clients and partners involved in our program and service evaluations. The survey is designed to be anonymous. Responses, including open-text comments, will be available to the Evaluation Division and leadership in the Perioperative program. Combined results may be shared with health care providers, evaluation partners, and leadership within NL Health Services. All information collected is stored in a secure office and/or on a secure network belonging to NL Health Services.
