Medical Imaging Patient Experience Survey


Purpose of the Survey
At Central Health, we are committed to providing quality care to patients and their families. Central Health is a Person- and Family-centered Care organization, which means working together with patients, families, health care providers, staff and volunteers to provide respectful, compassionate, quality and safe care.

Central Health has responsibility to clients, the general public, and the provincial government to ensure procedures and controls are in adherence to best practices and high standards in all aspects of business. Risks to participants who complete this survey are considered minimal. While there is no cost or benefit for participating, the data collected from this survey may be used for trending and tracking purposes with a goal of overall system improvement within Central Health.

This survey asks about your recent experience with Medical Imaging at Central Health. This is an opportunity for you to provide feedback. The results will be shared anonymously and only with relevant health care providers / staff within Central Health. Data collected for the survey will only be used for the intended purpose of quality improvement and will not be disclosed for any other purpose except as authorized by law.

This survey takes about 5 minutes to complete.
