NL Health Services - Central Zone is committed to providing quality care to patients and their families. NL Health Services is a people centred care organization, which means working together with patients, families, health care providers, staff and volunteers to provide respectful, compassionate, quality and safe care. This survey asks about your recent experience while admitted to a health care centre. This is an opportunity for you to provide feedback. The results from the survey will be shared anonymously with health care providers and the health region to improve the quality of services.

Anonymous responses, including open-text comments, may be available to the program leadership. Combined results will be shared with health care providers, leadership within NL Health Services and evaluation partners. All information collected is stored in a secure office and/or on a secure network belonging to NL Health Services.
