Virtual Emergency Department Patient Experience Survey

We welcome your feedback about your recent experience with the emergency services you received at a Western Health facility. This feedback will help improve the quality of emergency services and, more specifically, the use of virtual care in our emergency departments.

The survey takes about 5-10 minutes to complete. 
 
The survey is voluntary and will not affect your level of care. All information provided will be treated confidentially and reported collectively. This survey will remain open until sufficient responses are received to ensure adequate regional representation is received.  At that time, a summary of all the surveys collected will be shared anonymously with health care providers and leadership of Western Health. A summary report can be accessed by contacting Patricia Reid at patriciareid@westernhealth.nl.ca.
