Introduction
The Youth Evaluation Form is part of the reporting requirements related to the FNIYES Employment Strategy: Skills Link Program. The purpose of this form is to ensure that program delivery requirements are being met, that funds are expended on the intended purpose, and in accordance with the obligations ISC is accountable for.
The information collected through this form has been determined in conformity with the program guidelines and reporting guide issued by ISC.
Important Information to note:
1.Kindly submit a youth evaluation only for all the Mentored and ICT Work Placement
2.The Youth Evaluation Form is due by: Friday, March 22nd, 2024
3. The respondent who will sign the acknowledgement form (Pg. 4) will receive by email a copy of the form completed in PDF format for their records. Youth that completed the form should share a copy with the Band Education Coordinator.
4. The form has 17 questions that will take approximately 30 minutes to complete.
5. The "Save & Continue" is a feature in Alchemer that allows respondents to save & continue all the form progress up to that point is saved (including on the active page). Find this feature after the first page in the top-right of the second page, and proceed to enter your email address, within 5 minutes you will receive a link to return to your Evaluation later.
