Lung Cancer Experiences Survey: Help Shape the Treatments of Tomorrow

Canada's Drug Agency (CDA; formerly CADTH) is an independent, not-for-profit pan-Canadian health organization responsible for coordinating and aligning drug policy across provinces and territories. In other words, CDA is responsible for advising the federal, provincial, and territorial governments on which medications, procedures, and medical devices should be approved for use and reimbursed by government health insurance plans (For example, AHCIP, MSP, MCP, MSI, OHIP, RAMQ, etc.).

To make informed recommendations, CDA compares the data and economic model that pharmaceutical companies provide for a particular medication, procedure, or medical device (for example, from clinical drug trials) with the data in scientific literature, the observations from clinical experts, and the input patients, families, caregivers, and communities provide about their experiences with the same medication, procedure, or medical device. Patients, families, caregivers, and communities offer valuable insight on the diversity of individual needs and healthcare settings across Canada.

Lung Health Foundation, Lung Cancer Canada and the Canadian Cancer Survivor Network jointly prepared this questionnaire to compile the experiences of multiple patients, families, and caregivers into one report for the medications, procedures, or medical devices that will soon be coming up for review by the CDA.

We would like to thank you for agreeing to participate in this important survey. All information you provide will be held in the strictest confidence and will not be shared with third parties without your prior consent to do so or otherwise required by law.

The information you voluntarily provide in this questionnaire will be used to better inform our submissions to the CDA. Your responses will be combined with those of many others and summarized in a report to further protect your anonymity. No personally identifiable information will be shared within the submitted report.
