Medical Imaging - Primary Health Care Providers Survey

Introduction

As part of our commitment to improving Medical Imaging services within NL Health Services, we ask that you provide us with your opinion and comments about the delivery of our diagnostic and therapeutic services in which you refer your patients.

A few things to consider:

Taking part in this survey is voluntary
Your responses are anonymous and will only be reported in overall results, not individually
Your responses will be kept confidential, please see the Privacy and Confidentiality notice below for more details

Privacy and Confidentiality
NL Health Services prioritizes privacy and confidentiality for all participants, clients and partners involved in our program and service evaluations. The anonymous responses will be available to the Evaluation Department and leadership in the Medical Imaging Department. Combined results may be shared with health care providers, evaluation partners, and leadership within NL Health Services. All information collected is stored in a secure office and/or on a secure network belonging to NL Health Services.
