This survey should take no more than 15 minutes. Responses will be compiled to generate an Education and Practice Resource Asset Map for Canada. Results will be shared with the CCMG membership.

The CCMG Continuing Professional Development committee, in collaboration with the Canadian Rare Disease Network (CRDN), are identifying the educational and practice resources, tools and opportunities that you provide to non-genetics healthcare providers (HCPs). We are most interested to hear about:
- Genetics/genomics tools and resources contributed to a mainstreamed process or multidisciplinary clinic
- Practice resources developed for non-geneticist HCPs
- Educational activities or opportunities developed for medical students, and non-geneticist residents
- Educational opportunities provided to practicing non-geneticist healthcare providers
Definitions:
Practice resource – a tool/aid that is used in the clinic or to guide management
Educational resource – a tool/aid that is used to teach a HCP
We are aware that we may receive more than one response from your clinic. We will consolidate responses in any case.
