WCDSB Safe School Incident Reporting Form For Bullying and/or Hate-Based Incidents

The Waterloo Catholic District School Board (WCDSB) holds sacred the view that each child is created in the image of God, and that every person must be treated with respect and dignity. Every student has the right to be safe and to feel respected in and around the school.

The WCDSB encourages all incidences of safety concerns to be reported to the school principal. Recognizing that some individuals may not feel comfortable with direct reporting, the WCDSB has set up a safe and anonymous way to report. Students, parents/guardians, and Catholic school community members can use this tool.

The WCDSB Anonymous Safe Schools Incident Reporting Form is one way for students or community members to report a concern about a student’s safety. Parents and students are encouraged to report incidents where they feel their safety has been comprimised.  Participation is entirely ANONYMOUS and VOLUNTARY.  You will not be required to provide your name, contact information, or any information that can identify you unless you choose to do so.   As well, no identifying information (e.g., email address, IP address) will be automatically collected through the online tool.

All responses will be grouped together to support board and ministry level reports to evaluate progress towards MYSP goals and Ministry requirements. No individuals will be identified in any report. Information in this survey is collected under the legal authority of the Education Act (section 169.1 (1)) in compliance with the Municipal Freedom of Information and Protection of Privacy Act [MFIPPA]. In cases where there are fewer than 10 individuals belonging to a group (or combination of groups), responses will be further grouped to further maintain anonymity (e.g., combining groups of less than 10 individuals).
