This application form is for NEW research projects or proposals. Before completing this application form, please carefully read the OCREAC GUIDELINES. 
 
Researchers interested in viewing the application prior to completing the online form should use the following PDF copy. All sections of this application must be completed in full before your research will be considered for the review process.
 
Within this application you will be required to upload electronic copies of various documents. ALL documents should be combined into a single PDF document. Only ONE uploaded file is permitted. Once uploaded, the file cannot be deleted but can be replaced by another file.
