Employee Self-Screen for Respiratory Illnesses

Daily active screening is not required. Employees may use this tool if they have symptoms or have tested positive for a respiratory illness. You must answer both questions to determine if you are permitted to report to work today. 

Employees not permitted to report to work must notify their supervisor of their absence. 
1. Are you currently experiencing symptoms as listed below? Symptoms must be new or worsening and not be chronic or related to other known causes or conditions. Symptoms of a respiratory illness include:
  • Fever / chills
  • Cough
  • Sore throat
  • Shortness of breath
  • Runny nose / nasal congestion
  • Muscle aches
  • Headache
  • Extreme weakness and tiredness
  • Diarrhea or vomiting

2. Have you tested positive for COVID-19, influenza, RSV (Respiratory Syncytial Virus) and not yet discussed it with Human Resources. *This question is required.
