2026/27 Corporate Mentorship Program Profile Questionnaire

This profile questionnaire is used to support mentor-mentee matching for the 2026/27 Corporate Mentorship Program. It collects information similar to what was previously gathered through former mentorship website. You are asked to share details related to demographics, personal and professional milestones and accomplishments, corporate values, capabilities and skills, inclusion and diversity, and any preferences that will help a positive and meaningful mentorship experience.

As a mentee, please select your preferred mentorship format. If you formed a self-match partnership, choose self-match.

One-on-One Mentorship: A traditional format where one mentor is paired with one mentee.
Group Mentorship: Mentees will be placed in a group of three, guided by one mentor. Mentors choosing this option will work with a group of three mentees.
Either Format: You are open to participating in either a one-on-one or group mentorship format, depending on availability and matching. *This question is required.
