Part 1: Parent ID questions

Personal information collected on this form is collected under the authority of section 8(1) paragraphs 27.1 to 27.3 of the Education Act and Program/Policy Memorandum 170 in accordance with section 28(2) of the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). Personal information submitted through this form will be used to respond to and manage inquiries from parents and members of the school community. Have you already used this Student and Family support Office Form to address this same inquiry where a CASE ID # was provided to you?

Select "Yes" only if:

  • You are following up on a previously closed inquiry and have an existing case ID.
  • You were asked by a Case Manager to provide additional information or resubmit details related to an existing case.

Select "No" if this is a new inquiry:

  • We will treat this as a new inquiry and will not be able to review any previous submissions. Once you submit this form, we will send you a new CASE ID # by email.
