Clinical Implementation Tools Feedback Questionnaire
The Society of Obstetricians and Gynaecologists of Canada (SOGC) is collecting feedback to understand how its clinical implementation tools are being used and how they can be improved.

This questionnaire is part of the project’s quality improvement, monitoring and evaluation activities. It is not a research study. Responses are anonymous, and results will be reviewed in aggregate to inform future updates. Please do not include personal health information or information that could identify a patient.

Your feedback will help shape better education, resources and clinical tools to support trauma- and violence-informed, culturally safe, inclusive and patient-centred care in obstetrics, gynaecology and sexual and reproductive health.
