I agree that, if I am successful in the process, my nomination will be put forward for approval by General Council, and certain information about me (such as my photo and biography) will be made public at that time.

To apply for a leadership position, you must reside in Canada and maintain active CMA membership.

• Note: A 2027 CMA membership will be required in the new year for any candidates who wish to be considered for the next phase of the application process.
• For the position of Annual General Meeting chair, candidates must have a minimum of five consecutive years of CMA membership.
• If you are unsure of your CMA membership status, please confirm with our Engagement Support Team.

Annual General Meeting (AGM):

• AGM chair: bilingualism preferred; candidates must have a minimum of five consecutive years of CMA membership (three-year term, renewable once)

Please attest that you meet the following eligibility criteria for the AGM chair role: *This question is required.

• I have had an active CMA membership for the past five consecutive years.
• I would be available to attend the AGM and associated rehearsals in the Ottawa-Gatineau region in May/June each year of the term.

Please confirm below if you would like your application to also be considered should the following roles become available: *This question is required.

• I would like to be considered for the interim AGM vice-chair role (co-chairs the AGM and assumes all rights, duties and responsibilities of the AGM chair when the chair is absent and the AGM vice-chair role is vacant).
• I would like to be considered for the back-up AGM vice-chair role (the back-up is on standby to co-chair the AGM if the AGM vice-chair needs to step into the AGM chair role because the chair is absent).
• I do not wish to be considered for either of these roles.

Appointments Committee:

• Member from any province or territory who is not a CMA board director (three-year term, renewable once)

Awards Committee:

• Member from any region in Canada other than the following: Iqaluit (NU), Vancouver (BC), Toronto (ON) (three-year term, renewable once)

Please note that the selection timeline for this role will be from January to February 2027, rather than January to April 2027 as is the case for all other vacancies, as this role falls under the purview of the CMA board. The three-year term (renewable once) will begin in June 2027. The Awards Committee includes members from different regions across Canada. In 2027, the following regions will be represented: Iqaluit (NU), Vancouver (BC) and Toronto (ON). To be eligible for this role, you must reside outside of these regions.

Board of Directors:

• Alberta member (three-year term, renewable once)
• British Columbia member (three-year term, renewable once)
• Medical resident member (one-year term, renewable twice)

Committee on Ethics:

• Member from Alberta, Manitoba, Northwest Territories or Saskatchewan (three-year term, renewable once)

• Newfoundland and Labrador member (three-year term, renewable once)
• Northwest Territories member (three-year term, renewable once)
• Nova Scotia member (three-year term, renewable once)
• Yukon member (three-year term, renewable once)
• Medical resident member (three-year term, renewable twice)

Please attest that you meet the following eligibility criteria for the medical resident role(s):  *This question is required.

• I am currently a medical resident and will be enrolled in a postgraduate program at a Canadian medical school between Dec. 18, 2026 (the application deadline) and June 2028 (the end of the first one-year term in the medical resident role).

Please attest that you meet the following eligibility criteria for a jurisdictional role: *This question is required.

• I currently reside in the province/territory of the position, and I expect this to be my primary residence for the duration of my term if I am selected for the role (required under Bylaw 12.3.2).
