Health-Care Provider Survey: Mental Health & Primary Care Integration

Pour accéder à la version française de ce questionnaire, utilisez le menu déroulant dans le coin supérieur droit de votre écran.

In response to growing pressures on primary care providers to address mental health needs, the Mental Health Commission of Canada is exploring how best to strengthen the integration of mental health care into primary care across Canada.

We invite you to complete this brief survey to share your experiences and insights, which will help shape the Commission’s future priorities.

Your response will help identify what is working well, the challenges you face, and the support needed to advance integrated care across diverse health care settings.

The survey takes about 5 minutes; participation is voluntary, and responses will remain confidential and be reported only in aggregate form. Your insights are vital to strengthening mental health care across Canada.
