We present an unusual ovarian tumour arising in a patient with a complex clinical background. Histologic examination revealed two distinct but intimately associated morphologic components with contrasting immunophenotypic profiles. This case highlights the value of integrating clinical history, morphology and immunohistochemistry when approaching a diagnostically challenging ovarian neoplasm.
