Board Recruitment 2026-27 - Board Candidate Application

Introduction

In order to ensure diversity on the Board of Directors, the nominations committee is asking all candidates to identify what you believe you bring to the Centretown Community Health Centre (CCHC).

To run for a position on the Board of Directors you need to be a registered member of CCHC. Please make sure you complete the CCHC membership form, note that membership must be renewed on a yearly basis.

All questions must be answered.

1. I agree to be a candidate for election to the Board of Directors of the Centretown Community Health Centre (CCHC) for a two-year term from 2026 until 2028. *This question is required.
